Epidemiology and risk factors for hepatitis C virus infection in a high-prevalence population

To understand increasing rates of hepatitis C virus (HCV) infection in Tennessee, we conducted testing, risk factor analysis and a nested case–control study among persons who use drugs. During June–October 2016, HCV testing with risk factor assessment was conducted in sexually transmitted disease clinics, family planning clinics and an addiction treatment facility in eastern Tennessee; data were analysed by using multivariable logistic regression. A nested case–control study was conducted to assess drug-using risks and behaviours among persons who reported intranasal or injection drug use (IDU). Of 4753 persons tested, 397 (8.4%) were HCV-antibody positive. HCV infection was significantly associated with a history of both intranasal and IDU (adjusted odds ratio (aOR) 35.4, 95% confidence interval (CI) 24.1–51.9), IDU alone (aOR 52.7, CI 25.3–109.9), intranasal drug use alone (aOR 2.6, CI 1.8–3.9) and incarceration (aOR 2.7, CI 2.0–3.8). By 4 October 2016, 574 persons with a reported history of drug use; 63 (11%) were interviewed further. Of 31 persons who used both intranasal and injection drugs, 26 (84%) reported previous intranasal drug use, occurring 1–18 years (median 5.5 years) before their first IDU. Our findings provide evidence that reported IDU, intranasal drug use and incarceration are independent indicators of risk for past or present HCV infection in the study population.

INTRODUCTION
The grey wolf (Canis lupus) is the most widely distributed large carnivore in the northern hemisphere (Nowak 1995) and has a reputation for killing livestock and competing with human hunters for wild ungulates (Young 1944; Fritts et al. 2003). Wolves rarely threaten human safety, but many people still fear them. In the western USA, widespread extirpation of ungulates by colonizing settlers, wolf depredation on livestock and negative public attitudes towards wolves resulted in extirpation of wolf populations by 1930 (Mech 1970; McIntyre 1995). By 1970, mule deer (Odocoileus hemionus), white-tailed deer (O. virginianus), elk (Cervus elaphus), moose (Alces alces) and bighorn sheep (Ovis canadensis) populations had been restored throughout the western USA while bison (Bison bison) were recovered only in Yellowstone National Park. However, grey wolves were still persecuted. In 1974, grey wolves were protected and managed by the US Fish and Wildlife Service under the federal Endangered Species Act of 1973.
In 1986, the first recorded den in the western USA in over 50 years was established in Glacier National Park by wolves that naturally dispersed from Canada (Ream et al. 1989). Restoration of wolves in that region emphasized legal protection and building local public tolerance. Wolves from Canada were reintroduced to central Idaho and Yellowstone National Park in 1995 and 1996 to accelerate restoration (Bangs and Fritts 1996; Fritts et al. 1997). The Northern Rocky Mountains wolf population grew from 10 wolves in 1987 to 663 wolves by 2003 (US Fish and Wildlife Service et al. 2003) (Fig. 21.1, Table 21.1).

Surface films
Sources, sinks, and properties of surface films
Sea-surface films are derived from multiple sources, both in the sea and on land. Even in oligotrophic waters where biological productivity is low, the concentration of dissolved organic matter, including degraded biopolymers and geopolymeric materials, is sufficient to produce surface enrichments of organic matter under favourable physical conditions. Specific inputs from phytoplankton blooms and from neuston indigenous to the microlayer also contribute to the enrichment of surface-active matter at the interface. Terrestrial sources include both natural and anthropogenic contributions. Terrestrial plant-derived materials are released directly to the atmosphere and introduced via dry and wet deposition, or enter the ocean environment via riverine inputs as decay products of vegetation. Anthropogenic contributions include point sources related to industrial processes, agricultural runoff, and spills of petroleum products (catastrophic and chronic). In addition, municipal wastewater discharges are frequently highly enriched in surfactants that ultimately enter coastal seas and sediments. In shallow coastal environments, resuspension of sediments and release of sediment pore-water materials are other potential sources of surfactants.
The relative importance of these sources is not known. A major source of surfactants is thought to be production by phytoplankton, which exude natural surfactants as metabolic by-products (Zutic et al., 1981).
